Amen are recruiting for a Social Care Team Leader.  For details on the position and how to apply please see below: Job Description of:  Full Time Social Care Team Leader Reports to:  Manager Role:  to be responsible for the provision of services to Clients and to coordinate the team effort to provide an effective service.  To always promote Amen Support Services CLG in a positive and professional manner. Specific Duties and Responsibilities of this Role:
  • To coordinate and lead in the input of Client Information and Care Plans onto the Amen client database ZOHO by the team
  • To facilitate a Pattern Changing Programme for Men who have experienced Domestic Abuse in consultation with the management team
  • To be the point of contact for the Manager & Finance & Administration Officer for the collection and collation of statistics
  • To act as a Keyworker for assigned Clients and through completion of a Holistic Needs Assessment with the Client, to agree and implement a Personalised care Plan.
  • To oversee the implementation of Personalised Care Plans and be the point of contact for the Manager in regard to the clients of Amen
  • To log details for clients on to CRM Database.
  • To maintain accurate case notes on all work carried out and to oversee the completion of all assigned client case files.
  • To work with the Manager and other team members to review the needs of clients on a regular basis.
  • To liaise with other Staff to ensure follow-up on queries when they are not on duty.
  • Support of other members of the team.
  • To continually review the quality of service with the Manager, fellow team members and clients’ accompaniment Service for clients requiring emotional support.
  • To work closely with the Accompaniment & Outreach Workers and as necessary when available, to assist them with their work.
  • To highlight the issue of violence and abuse against men amongst voluntary and statutory services.
  • To work as part of the team with in Amen Support Services CLG, focusing on the issue of domestic abuse and male homelessness and promoting the organisation in a positive and professional manner.
  • To network with local based community groups, organisations and statutory voluntary agencies to create the awareness of the services offered by the organisation and facilitate referral to the service.
  • As part of Amen Support Services CLG team, to offer information on a range of options available to men who are experiencing abuse in their personal relationships or homelessness.
  • As part of Amen Support Services CLG team, to be available to listen to and support clients in identifying their needs and working with them to address these needs.
  • As part of Amen Support Services CLG team, to facilitate Clients to access services and supports from relevant agencies and to advocate on their behalf where appropriate.
  • To maintain written records of work completed and submit a monthly progress report to the Manager
  • To complete own Weekly Time Sheets for authorisation by the Manger and complete necessary forms for Travel Expenses, Annual Leave, etc.
  • To attend Full Core Staff Meetings and Support Staff Team Meetings and to attend Clinical Supervision every 6 weeks.
  • To attend Training as requested by the Manager.
  • To provide Statistics and other information, as requested from time to time by the Manager, for Grant Applications, etc.
  • To comply with responsibilities of the Employee, as set out in the company’s employee handbook and all Policy / Procedures.
  • Maintenance of helpline staff roster, to ensure that cover is maximized at all times. This includes carrying a mobile phone on a call basis to ensure that any last-minute absences/changes can be addressed while the service is operating, so as not to affect the running of the service.
  • To liaise with other staff to ensure follow-up on queries when you go off duty.
  • To continually review the quality of service with the manager, fellow team members and clients.
  • To strictly comply with our confidentiality policy and data protection policy and to forward all data requests to the manager
  • To seek approval from the designated liaison person to forward Child Protection or Welfare Concerns to TUSLA
  • To attend Reflective Practice Meetings once a month and Staff Team Meetings as necessary and to avail of the Clinical Supervision provided every 6 weeks.
  • HETAC / DIT Degree/ National Diploma in Social  Care
  • HETAC / DIT Degree /National diploma in Applied Social Studies.
  • RNID Nursing Professional Qualification.
  • Teaching degree, Youth & Community Studies, Psychology and Social work,
Preferred: Certificate in the fundamentals of understanding and responding to domestic abuse level 8
Experience working as part of a multidisciplinary team.    
  1. Minimum of 8 years’ experience with one or more of the above qualifications and working in the area of Domestic abuse (Men / Women)
  2. Frontline experience in addiction, mental health issues, self-harm, suicidal tendencies
3.     Experience in liaising with homeless / residential services, addiction services, mental health services
  1. Key working specific clients, devising and delivering individual holistic care plans
  2. Working with ethnic minority’s / migrants demographic
  3. Outreach work in clinics or clients homes
  4. Court accompaniment, family and criminal court.
  5. Accompanying clients to hospital, GP
  6. Liaising Gardai to clients homes when in a dangerous situation
  7. Tenancy sustainment
  8. Keeping up to date files on all residents and families
  9. Liaising with all local services
  10. Facilitating group meetings
  11. Working with children who suffered or witnessed domestic violence
